Multiple Issues: Contact Page and Mobile Version

Hi there,

I am working on customizing this theme for my client and have run into a few issues. This is the website URL: http://betasite.xsolcorp.com/

1) Contact Page Form: The form on the Contact Page does not display like the one on the demo website. Are there settings for this? Or is this missing some CSS code? (screenshot attached)

2) Contact Page Map: I received the API Key from the client, and input it, but it does not display. Have I done this correctly? (screenshot attached)

3) Mobile view: The mobile view of the whole site is not displaying properly. For example, the slider is cut off significantly on the homepage, there’s is no navigation/menu at the top, etc. I think there’s an issue with the Header portion of the template builder? Could you look into this? (screenshot attached)
